If you have not completed a Medical History form in the last 6 months, then a new form will need to be completed. If you are not currently under treatment or have not attended the practice in over two months, then a new NHS PRW form will need to completed. Our reception team will aim to send your paperwork to you via an email address we hold on your file; it is a great help if these are completed ahead of arriving for your appointment. Alternatively, your forms can be completed upon arrival for your appointment, in which case we kindly ask that you arrive 10-15 minutes early.
